ABSTRACT

Comparative international studies show that about half of the deceased in the COVID-19 pandemic were persons living in institutional and residential eldercare. As seniors are the most affected age group, we aim to study if and to what extent the eldercare services were included in the National Pandemic Plans, and how they were included in the response during the first phase of the pandemic in Finland, Iceland, and Sweden. We use the CRISMART approach to crisis documentation and analysis in comparing national response to the pandemic for the eldercare sector. The method enables comparison of extraordinary crisis situations from the decision-making and policy-making perspective. We found that there were both similarities and differences in the preparedness of the three Nordic countries, as well as in how they responded to the pandemic. In all three countries the focus of the national responses framed the problem as a health and healthcare services' problem. We also found value conflicts in the response between the value of protection versus social contact and self-determination and hence relating to the quality of eldercare. Keeping in mind the proportional increase of elderly people, care challenges, and future crises, we must strengthen the position of local social services within the emergency management systems to enhance disaster resilience and sustainability of our societies.

ABSTRACT

Prevention of noncommunicable diseases (NCDs) remains one of the main global strategic directions of the WHO activities. This situation is based on the present and predicted significant contribution of NCDs to the total morbidity and mortality of the world population related to an increase in the number of the older-age-group population, after 65 years, as well as the negative impact of risk factors, such as behavioral and psychosocial, infections and viruses, climate and environmental changes on the development, progression and outcomes of these diseases. The evidence-based confirmation of such processes has received the during the pandemic of the new coronavirus infection at the global level. The WHO also pays special attention to the growing problem of mental health associated with neurodegenerative diseases and neuropsychiatric disorders, as a consequence of the population life expectancy increase, influence of viral infections, climate and environment changes. At the same time, it is proven that mental disorders are factors that contribute independently of other causes to the progression of NCDs. In this regard, in order to significantly and long-term reduce the burden of NCDs, among the recommended global strategic directions for the prevention of NCDs, WHO will special focus on evidence-based economically justified and available interventions at the national or population level, outside of health systems, as well as in the frame of primary health care to ensure the universal health coverage. Effective approaches of such interventions will allow in the future save the population health, accomplish the WHO 13-th General program work tasks and achieve the UN Sustainable Development Goals by 2030, preserve and increase human capital, the well-being of all people throughout the planet. (English) [ FROM AUTHOR] Профилактика неинфекционных заболеваний (НИЗ) остается одним из глобальных стратегических направлений деятельности Всемирной организации здравоохранения (ВОЗ). Данное положение основано на существующем и прогнозируемом значительном вкладе НИЗ в общую заболеваемость и смертность населения мира, обусловленном увеличением численности населения старших (после 65 лет) возрастных групп, ростом негативного влияния поведенческих и психосоциальных факторов риска, инфекций и вирусов, изменения климата и окружающей среды на развитие, прогрессирование и исходы этих заболеваний. Доказательное подтверждение происходящих процессов получено на глобальном уровне в период пандемии новой коронавирусной инфекции. Пристальное внимание ВОЗ обращено и к проблеме ухудшения психического здоровья на фоне роста частоты нейродегенеративных заболеваний и психоневрологических расстройств вследствие увеличения продолжительности жизни населения, влияния вирусных инфекций, климата и окружающей среды. В то же время доказано, что психические расстройства являются независимыми от других причин факторами, способствующими прогрессированию НИЗ. В связи с этим для существенного и долгосрочного снижения бремени НИЗ среди рекомендуемых глобальных стратегических направлений профилактики этих заболеваний ВОЗ особо выделяет научно обоснованные, экономически оправданные и доступные мероприятия, которые следует осуществлять на общенациональном или популяционном уровне, вне систем здравоохранения, а также в рамках первичной медико-санитарной помощи, обеспечивая всеобщий охват услугами здравоохранения. Эффективность выполнения таких мероприятий позволит сохранить здоровье населения, выполнить задачи 13-й общей программы работы ВОЗ и достичь заявленных целей устойчивого развития ООН к 2030 г., сохранить и увеличить человеческий капитал, благосостояние всего населения планеты. ABSTRACT

Introduction: the current epidemiological situation in the world, characterized as a pandemic since March 11, 2020, has proved the importance of developing national health systems, and also emphasized the need to develop the channels of international cooperation in health care for joint operational measures to combat the spread of the virus. In this regard, the purpose of the study was to determine the prospects for the development of this area and the opportunities for improving the existing systems. Methods: the general scientific methods were used in the research process: logical, system-structural, complex, and functional, as well as the specific scientific methods in the form of the method of concrete-sociological research, the method of modeling, and the formal-legal method. Results: the paper considers the basics of the implementation of international cooperation of the Russian Federation in healthcare amid the pandemic: the differentiation of international cooperation formats is carried out, the legal bases of cooperation are analyzed, and the organization of interaction with the World Health Organization is studied. Justified in the work the trajectories of the development of international cooperation in healthcare are predicted. Conclusions: as a result of the study, it is established that the post-coronavirus era will be characterized by new priorities for cooperation. On the basis of the conducted research, a proposal was made to amend the Federal Law “On the Basics of Public Health Protection in the Russian Federation”, as well as the main recommendations for further implementation of international cooperation in healthcare.Alternate : Введение: современная эпидемиологическая обстановка в мире, характеризуемая с 11 марта 2020 г. как пандемия, доказала важность развития национальных систем здравоохранения, а также подчеркнула необходимость развития каналов международного взаимодействия в сфере здравоохранения в рамках принятия совместных оперативных мер для борьбы с распространением вируса. Ð’ связи с этим целью исследования стало определение перспективы развития данного направления и возможности для совершенствования существующих систем. Методы: в процессе исследования были использованы общенаучные (логический, системно-структурный, комплексный и функциональный) и частнонаучные (метод конкретносоциологических исследований, моделирования, формально-юридический) методы. Результаты: в статье рассмотрены основы реализации международного сотрудничества Российской Федерации в сфере здравоохранения в условиях пандемии: проведена дифференциация форматов международного сотрудничества, проанализированы правовые основы сотрудничества, изучена организация взаимодействия с Всемирной организацией здравоохранения. Спрогнозированы охарактеризованные в работе траектории развития международного сотрудничества в сфере здравоохранения. Выводы: в результате исследования установлено, что для посткоронавирусной эпохи ±ÑƒÐ´ÑƒÑ‚ характерны новые приоритеты сотрудничества. На основе проведенного исследования высказано предложение внести изменения в Федеральный закон «Об основах охраны здоровья граждан в Российской Федерации», а также представлены основные рекомендации дальнейшей реализации международного сотрудничества в сфере здравоохранения.

ABSTRACT

Coronavirus outbreak in Wuhan, China, turned into a pandemic in record time. Communication of disease presentation and mechanism of spread remain keys to getting ahead of the virus and limiting its spread beyond the capacity of management. Owing to huge academic focus and pandemic concern around the globe, this bibliometric analysis investigated research productivity related to coronavirus disease (COVID-19) pandemic using the Web of Science database. The relevant data were harvested, and search query was further refined by publication years (2020 OR 2019) and document types (article, book chapter, and proceedings paper). Finally, 6694 records were imported and downloaded in Plaintext and BibTeX formats on August 1, 2020. The data analysis was performed using MS Excel, VOS viewer, and Biblioshiny software. Of the 6694 publications that appeared in that period, the USA and Chinese research institutions topped the numbers. At the same time, the Journal of Medical Virology and CUREUS (Cureus Journal of Medical Science), remained favorite journals for publications. The pattern of multi-author publications has outstripped that of single-authors. Apart from COVID-19 and the novel coronavirus, the important keywords mentioned included pandemic, pneumonia, epidemiology, public health, outbreak, epidemic, China, infection, and treatment. The analysis shows a strong local research response from China, with large teams reporting on the disease outbreak. Subsequent studies will document a global response as the virus spreads worldwide. The initial research related to the current coronavirus outbreak was reported from within China. The data and patterns were supposed to alter as the virus spread globally.
